 ACU-Track Patient Outcome Plans

Your ACU-Track account includes a series of Plans to help you monitor your patient’s specific health complaints. You will see these Plans displayed under the ‘Patient Outcome Plans’ button in each patient’s Clinical Record underneath the Pathway name.

Once initiated, these plans will open a baseline questionnaire form immediately, and automatically send all follow-up questionnaires to the patient via email at pre-defined time-points of:

  • 2 weeks

  • 4 weeks

  • 8 weeks

  • 12 weeks

  • 6 months

  • 9 months

  • 1 year.

These plans make it as easy as possible for you to monitor your patient outcomes for a wide range of health complaints, and ensure that your results are easily comparable across all your patients.

 

Pain ONLY Plan

The ‘Pain ONLY Plan’ is ideal for any type of pain condition, especially when pain is the patient’s main issue, but they are otherwise healthy and well. It includes a simple 0-10 pain question, and is quick and easy to complete.

Wellbeing and Pain Plan

The ‘Wellbeing and Pain Plan’ is more appropriate to use when the patient is presenting with a more complex or long-standing health complaint, which is affecting their general wellbeing and quality of life.

Or, if you are focussing more on improving overall patient health and wellbeing, rather than treating a specific problem.

It includes a simple 10 question global health questionnaire which tracks overall physical and mental health, including wellbeing, energy levels and also includes a 0-10 pain scale.

Mental Health Plan

The ‘Mental Health Plan’ is ideal for monitoring patients presenting with any kind of psychological or mental health issues.

It uses two brief questionnaires to assess key symptoms of anxiety and depression.

The ‘Mental Health Plan’ can also be used when there is a clear element of anxiety or depression accompanying another health complaint, such as chronic pain. In this case you could initiate both the ‘Pain Only Plan’ and the ‘Mental Health Plan’.

 

Patient Improvement, Satisfaction and Experience

All Plans include a retrospective question in all follow-ups, ‘Patient’s Health Complaint Follow-up’, which asks the patient how their health complaint has progressed overall, compared to when they first started their course of treatments or care.

It also asks the patient to provide any additional details about anything else in their lives that may also have affected how their health complaint has progressed.

At 8-weeks, patient’s will also be sent a one-off additional follow-up questionnaire, asking them how effective they felt their care was in addressing their health complaint, their overall satisfaction, any adverse events they experienced.
